Still stuck at the following error:
"Did not find a usable MAC Address for Entity ID. Please file a bug report at https://bugreporter.apple.com with the model identifier, IOConsoleUsers: time<0> 0->0"

Actually if you do this correctly there wont be an error, you can check if the boot-flag is active by visiting Clover Options menu.

While Installing, You should never select MakeMacWork from Customize section, also don't forget to unselect Backup Graphics Extensions.

I do not use MakeMacWork and I did unselect Backup Graphics Extensions. The PCI Configuration Begins is okay now, it's this Entity ID error that keeps stopping me cold.

Do you mean there is a kernel panic after the line

Your graphics card is not being detected or Graphics Injection is not working.
Try to boot with -radvesa bootflag

Reinstall, Select Backup Graphics Extensions while Installing.
To replace Graphics Extensions you must use the command.
sudo grafix replace all
You can add the npci=0x3000 into boot arguments by using Clover Configurator.
